Care coordination is a strategy that has the potential to improve the

effectiveness, safety & efficiency of the American healthcare system.

This series is designed to complement several other trainings for the ECQIP project with CHAD to assist FQHCs in building successful care coordination programs. It aims to provide role clarity that will increase awareness and understanding of how various team members play a role in care coordination and how workflow optimization can assist FQHCs in designing and implementing successful care coordination programs by improving patient engagement.
