This ECHO series creates a virtual space where primary care providers, health professionals and specialists can collaborate to gain skills and understanding regarding how to address health literacy to improve the patient experience and outcomes for individuals with diabetes. The four-session series will provide practical information related to the current literature and basic health literacy practices, and tools that can be used to measure a patient's health literacy. It will also address how health literacy can impact patient outcomes. Learn from experts in the field about incorporating these tools into your practice in order to gain confidence and improve care for your patients.

Addressing Health Literacy in Diabetes Care and Prevention

Audience:
Health care professionals who care for people living with diabetes and/or deliver Diabetes Self-Management Education and Support (DSMES) and the National Diabetes Prevention Program (NDPP). This includes registered dietitians, pharmacists, nurses, PAs/NPs, certified diabetes care and education specialists (CDCES), health coaches/trained lifestyle coaches, community health workers, social workers, etc.
