This ECHO series is designed to support providers, clinical care team members and non-clinical staff who work with patients managing cardiovascular disease. Over the course of four sessions, participants will learn about and discuss tools for the implementation of eHealth modalities and digital inclusion for improving patient management of cardiovascular disease. Each session will include a brief presentation by an expert followed by an interactive discussion.
